Preparation of New Civil Pensions
After receiving the pension file from the institution where the pensioner last worked,following documents should be present to prepare the pension.
- Retired Identity Card and Treasury 70
- Residence certificate obtained from the Grama Niladhari of the relevant area
- Photocopy of the pass book of bank account
- Retirement declaration
Preparation of Widows' Pensions
After we receive the death gratuity file, informing office to terminate the civil pension for the deceased, family should hand over a widows' pension request letter with a photocopy of the death certificate and W&OP number The following documents should be submitted when we receive a letter from the Widows and Orphans Division of the Department of Pensions stating that we have sent the file to the Divisional Secretariat..
- Original Certificate of marriage
- An affidavit in case of difference in name
- Original copy of death certificate
- A copy of the pensionable salary slip
- Birth certificate of children under 21 years
- 3 photographs(3 ¼, 2 ¼ size) with Grama Niladari recommendations
- National Identity Card
- A letter obtained from the relevant divisional secretariat that the pension is terminated
- Widow / Widower's birth certificate
Also, Form 15 of the Retirement Widow's Fund and the letter to be submitted to the Grama Niladhari should be obtained from our office.
Preparation of Orphans' Pension
When a letter has been sent to the office from the Department of Pensions, the widow orphan' should come to our office with the following document
- Birth certificate of the orphan
- Originals death certificates of mother and father
- Marriage certificate of mother and father
- If children less than 18 years should come with a guardian who is above 21 years, 5 / 1, 6,7,8 applications should be submitted.
Preparation of Disability Pensions
Divisional Secretariat should be informed to terminate the pension or widow's pension in the month of death occurred and following applications should be submitted.
- orphan children application for the permanently disabled
- Affidavit of the disabled Child's guardian
Applicant must arrive at the office on the day and time of the notice for the initial examination of the defect. If a guardian is needed, he/she too should also be in the preliminary examination.
Paying for the Heir
- Make our Divisional Secretariat aware to stop the pension or widow's pension
- Original Death Certificate
- A letter signed by a Justice of the Peace with their names and signatures stating that the other heirs have no objection to the payment of that amount to one. (married and unmarried)
- Grama Niladhari report including names of the heirs of the family, the relatives of the deceased, age, sex, and family details.( it should be recommended by Divisional Secretary)
- Form General 77
- As this amount is paid by check, please state your bank account number and the name of the bank
