8. Grama Niladhari Residence certificate

10. A photo of business certified by the Grama Niladhari
11. If there is a change in the name of the applicant, the following documents should be submitted
    1. Copy of NIC
    2. Affidavit
12.  If the business is located on undivided land, the following documents should be submitted
    1. Letter of consent from all land owners
    2. Deed plan
